I acknowledge, agree, and understand that The Carbon Collective provides soft tissue work, which may include but is not limited to massage, myofascial release, trigger point therapy, cupping, and other related modalities. I understand that the purpose of this work is to address musculoskeletal discomfort, improve mobility, and promote relaxation. I understand that soft tissue work, while generally safe, carries potential risks, including but not limited to temporary soreness or discomfort, bruising, skin irritation and in rare cases, nerve or vascular damage. I understand that it is my responsibility to inform the clinician of any changes in my medical condition. I understand that my personal and medical information will be kept confidential in accordance with applicable privacy laws. I hereby release and discharge The Carbon Collective, its clinicians, employees, and agents from any and all claims, liabilities, and demands arising out of or related to the soft tissue work provided, except for claims arising from gross negligence or intentional misconduct.
